To administer any medication you need to fill out the Medication Authorization Form (under Enrollment Information / Forms tab).

 

This form is valid for 30 days. All medication is stored in a locked container away from children. Medication is administered according to instructions given on Medication Authorization Form.

 

If your child needs medication while in my care, give the morning dose at home. All medication to be given must come in the original labeled bottle; prescribed medication must have the child’s name on it and the instructions of distribution.
